Attention Interpreters: At the end of the assignment, please complete the Access Verification Form as required for contracts with the State of Arizona.
When an assignment requires verification, the confirmation email will include a Verification of Service (VOS) link in the Session Notes section. Whenever possible, please use the link provided, as it is the easiest and most convenient way to submit your verification. Completed VOS forms using the link are automatically emailed to the right address after they are signed. Not all jobs require a VOS.
If you have been requested to complete a VOS for an assignment and do not see a link in your confirmation email, please use the Access Verification Form below.

You can submit the form in one of three ways:

Alternatively, you can print the form, have it signed by an authorized person, and take a clear photo of the signed document.
